FRIEND REQUEST: One friend request of the SAME GRADE can be indicated below. Both friends must request each other on their form. Please understand that we cannot guarantee same-class placement. We will make every effort to place your child with the friend request submitted on their form, but there is no guarantee. For privacy and safety, we will not publish names with photographs/videos.
I authorize the use/release of photographs and/or videos that include the INDIVIDUALS NAMED ABOVE for University Carillon use, in print and electronic materials (worship videos, e-mail blasts, church website, Facebook, etc.).

All families should be aware of COVID-19 symptoms and should remain at home if they feel sick or have been exposed to someone who is sick. We love your children/students and want them to come…but more than that, we want your family to stay safe and healthy!

Masks will be welcome and encouraged but not required, and strongly encouraged for anyone not fully vaccinated. Given the fluidity and changing landscape of COVID, we will keep an eye on case numbers, emerging variants and CDC health guidelines. If best, we will adapt protocol as needed and keep parents and children/students updated as we do.

If you register but become unable to attend due to the impact of COVID-19 or protocol shifts, please reach out to sports@universitycarillon.net right away. We will work with you to refund registration costs.
